The Story:

On August 29th, 1990, a C-5 Galaxy was departing Ramstein Air Base in support of Desert Storm. With a Reserve crew from the 433rd AW from San Antonio, TX at the controls, the aircraft suffered from a deployment of a thrust reverser on one engine. The aircrew stood little chance in correcting the matter so shortly after takeoff.

17 people were killed in the crash but 4 survived. See this website for more information and historical photos.
